If you’ve been off sick for more than 7 days
To request a sick note:
- fill out a first sick note request form
- phone us on 0161 870 8099 or visit the surgery from 10am to 6pm
- contact the doctor responsible for your care if the sick (fit) note is related to a hospital procedure such as an operation as hospital doctors are required to provide sick notes in this instance
It is not normally necessary to see a doctor before your return to work at the end of the sickness period and you can go back to work sooner if you feel well enough.
The doctor may call you or arrange for an appointment. If the doctor has enough information to issue a sick note, we will try to have this ready for collection within two working days.
Extending your sick (fit) note
You can request an additional sick note if your existing one is due to expire.
The doctor may call you or arrange for an appointment.
If the doctor has enough information to issue a new sick note, we will try to have this ready for collection within 5 working days. Please note that Fit notes can be back dated so any time off will be accounted for.

If you’ve been off sick for 7 days or less
If you have been ill for 7 days or less you don’t need to see a doctor. You can complete a Self Certification form yourself. However some employers insist on a doctor’s note regardless. A doctors note under these circumstances involves a charge.
Many employers have their own self-certification forms. If your employer doesn’t have its own form your can download this Self Certification Form (PDF). Please print it, fill it in and hand it in to your employer. You do not need to see a doctor.
